PLEASE CONSIDER BEING A TEAM CAPTAIN FOR THE PARTNERS SUPERSTARS EVENT JUNE 12 & 13 2010 AT LINCOLN PARK

 

PARTNERS SUPERSTARS TEAM CAPTAIN’S MEETING March 1st 2010.  TEAM PACKETS WILL BE AVAILABLE March 1st 2010.

  

We hope that you will consider participating in the 2010 Partners Superstars Fundraiser.

 

Team Packets/Superstars Posters will be available on March 1st 2010More tickets available if desired.

 All events are at Lincoln Park, including the Barbeque at 1:30pm Sunday, June 13th

 

 

  1. Teams Consist of up to 15 total members and at least 4 must be females:

n  All players must have filled out insurance waiver forms which need to be submitted by Friday, June 4, 2010.

n  You can substitute freely among the 15 persons on your roster. All team events require at least 10 persons of which at least 4 must be females.

n  All forms may be faxed to Partners at 245-7411 by June 4, 2010.

 

2.    Entry Fees/ Ticket Sales/Advertising Books

n  $1,000 entry fee per team.  We ask that $500 be raised by Friday, April 30, 2010 to let us know that your team is for real.

n  One point reduction for every $10 below the $1,000 fee as of 4pm on Saturday, June 12th.

n  One bonus point for every $10 over $1,200 as of 4pm on Saturday, June 12th.

n  Advertising sales forms for Superstars Ad Book. Due no later than Friday, April 30, 2010.  Please turn in ad order forms as you sell them so we can start on the book as soon as possible.

3.        T-shirts: Need T-shirt order form for your team by Friday, April 30, 2010.

 

4.      Deadlines: Friday, April 30th 2010. T-Shirt Order for your team and Order Forms for Ads and half of entry fee.

 

                       Friday, June 4th: Team Roster Forms and Insurance Waiver Forms.

Turn in last of money by 4pm on Saturday, June 12th.
